Choosing skilled nursing or rehabilitation care is a big decision. Our team will walk you through each step, answer your questions and help you understand your options.

How admission works

  2. Apply. Complete the residency application (fillable PDF). For short-term rehabilitation, your hospital case manager can send a referral. [PLACEHOLDER: confirm referral process]

Paying for care

Rehabilitation services are generally covered for older adults eligible for Medicare reimbursement. The Cottages also accept patients with private insurance or the ability to pay privately.

Looking for placement for long term care?

Type into the application on your computer and print it, or print it blank and fill it in by hand. Sign it and return it in person, by mail, or by fax to 315-571-0800.
